Performing the rhinoplasty

The procedure is challenging due to the complex structure of the nose. The nose consists of several cartilages and bones that are closely dependent on each other and the skin structure to form the external aspect. Various structures often have to be altered, even in the case of minor rhinoplasty. At the beginning of this chapter we will go into the basic principles and later we will highlight specific points.

The nose hump correction

Frequently asked questions about nose correction

When it comes to aesthetic correction of the nose, many patients come to us to have a hump removed. As the nose in the center of the face has a significant impact on the aesthetic appearance, a nasal hump can unfortunately quickly catch the eye in a negative way. The strong prominence of the nose or hump can be a problem, especially for people from the Turkish-Arabic region, with whom they do not feel comfortable.

What is our procedure for removing a nasal hump?

Bony or cartilaginous protrusion of the nasal roof leads to the formation of a nasal hump. If it is only slightly pronounced, we can reduce it with the help of a bone rasp. A particularly careful approach is necessary here, as the bone of the nasal roof varies in thickness and our plastic surgeons must adapt the reduction to the bone conditions.

Medium / large nasal hump

Removing a medium-sized or large nasal hump is a different matter. We use a hammer and chisel for this.

Closed rhinoplasty

If there are no contraindications, we can perform a closed rhinoplasty. To do this, we first cut the cartilage of the nasal roof. We insert a chisel through this incision and use it to remove the excess bone in this area. We then remove the loosened bone and the nasal roof is exposed. Plastic surgeons also call this condition an “open roof“. However, as the roof needs to be closed again, we move the lateral bone parts towards the midline in a further step. To mobilize these bone parts, the base is divided with the chisel so that the two lateral pieces can be brought together. The roof is closed again.

Open rhinoplasty

Another option is open rhinoplasty, where our plastic surgeons can perform the operation under visual control. The advantage in this context is that we can also perform an exact correction of the tip of the nose in this way in Düsseldorf. Our Düsseldorf team inserts a very small chisel through millimeter-sized incisions in the skin and breaks the bone without breaking through it. The bone can then be bent. As the procedure is carried out gently and not all layers of the bone are cut through, the nose is relatively stable after the procedure.

… we can build up the bridge of the nose by up to several millimeters. Even if irregularities have occurred as a result of accidents or previous surgery, the filler offers an option for nose correction without surgery.

Minor asymmetries

…can be concealed without surgery by injecting some hyaluronic acid into the nasal bridge and correcting the nasolabial angle.

Small nasal hump

We inject some hyaluronic acid over the hump of the nose (in the hollow between the forehead and the bridge of the nose). This makes it less noticeable.

Please note:

With the rhinoplasty without surgery however, it should be borne in mind that this does not lead to a reduction, but rather to a build-up of the nasal structures. The nose is contoured in the process. Furthermore, the effect of the treatment is of limited duration.

The shelf life is 12 to 18 months, depending on the type of hyaluronic acid.

What is rhinomegaly?

In medical terminology, the diagnosis that makes a nose reduction appear sensible if the patient is suffering is called rhinomegaly (large nose) and attracts a lot of attention. Even an attractive face can appear unattractive due to a very large nose. In the majority of cases, a large nose is inherited and can also be caused by illness.

For this purpose, a nose reduction is performed as standard at Yuveo in Düsseldorf.

How is the nose reduction performed?

The operation always requires general anesthesia.

  • Lowering and narrowing of the bony and cartilaginous nasal roof
  • Shaping and, if necessary, reduction, raising of the tip
  • As a large nose often also means large nostrils, these often have to be reduced in size in order to achieve an attractive result.

As the tip usually also has to be corrected during a complete nose reduction, we usually perform the operation as an open rhinoplasty.

A small W-shaped incision is made on the nasal bridge between the nostrils in order to be able to operate openly on the tip. This facilitates the exact placement of sutures and enables precise incisions to be made on the cartilage.

In the case of rhinoplasty of a nose that protrudes far out of the face, the nostrils can sometimes appear extremely large after the nasal roof has been lowered.

There is also a correction option here: nostril correction.

A wedge is removed at the base of the nostril and the resulting defect is sutured in such a way that the scar is barely visible later on. We pay particular attention to symmetry and stability when suturing, as there is a weak point here with regard to blowing the nose and blowing the nose in the first 2 weeks after the operation in Düsseldorf.

The type of follow-up treatment cannot be generalized.

Tamponades

In an aesthetic procedure, it is not necessary to tamponade the entire nasal cavity unless a major correction of the septum or turbinates has been performed. Nasal plaster

If the bone had to be broken during rhinoplasty, a nasal plaster cast, which is usually left in place for 2 weeks, is essential. However, a splint is often applied to reduce swelling even without a correction to the bone. In milder cases, plaster staples are sufficient. Today, plastic plaster is usually used, which is fixed to the nose with a plaster bandage.

Threads

After an open rhinoplasty, the stitches on the nose bridge are removed after around 7 to 10 days at a check-up appointment at our clinic in Düsseldorf.

Behavior

As already described, an elevated upper body posture is beneficial after cosmetic nose surgery. Cooling can reduce the swelling of the eyelids. Blowing your nose should be avoided as this can have a negative effect on the surgical result. If you do blow your nose or sneeze, do so through your open mouth to reduce the pressure. When blowing your nose, avoid wiping at first – dabbing is gentler. Mobilization of crusts can lead to renewed swelling and bleeding. Talking too much is also unfavorable in the first few days after aesthetic surgery. It is helpful to protect the mimic muscles.

Sport

Initially, the nose is still very sensitive to impact and the bone is not yet fully stabilized. For this reason, contact sports and ball sports in particular should be avoided for 4 to 6 weeks after a cosmetic nose operation.

Nose shaping

The shaping of the nose is a long-lasting process, most of which is certainly completed after about 3 months. After that, only minor changes can be seen. In the case of very large or long noses, a lot of patience is required after an aesthetic nose operation with regard to the shrinkage of excess skin.

CostsThe costs for a nose correction are extremely variable. The following factors play a role here:

  1. How extensive is the rhinoplasty? Is only a minor correction performed, e.g. a nose tip correction, or a complete nose reduction? Does the nasal septum need to be operated on? Is it possible to achieve a useful result with an injection without surgery?
  2. Duration of hospitalization
  3. Costs for general anesthesia or twilight sleep (for minor surgery)
  4. The plastic surgeon who treats you.

The cost of a nose correction depends on the extent of the nasal deformity and the associated surgery.

In the case of a minor rhinoplasty, e.g. only the nostrils, the costs are considerably lower than for a complete rhinoplasty, in which not only the outer nose including cartilage and bone is changed, but also the turbinates and nasal septum are operated on.
