The auricle and its size play an important role in ear surgery. It becomes more complex if the auricle has a large base (concha). Depending on the size of the pinna, the following options are available:
Ear placement for slightly enlarged concha
The surgical method is supplemented by one or two strong sutures between the base of the pinna and the periosteum on the process of the temporal bone (os mastoideum, mastoid). The latter is the bony bulge that can be felt behind the ear. It is called the mastoid process in German.
The suture allows us to rotate the auricle in the direction of the bone (concha rotation).
Cartilage removal is performed in the case of a large auricle (concha).
The method of ear placement with the removal of a cartilage part is selected by us for large auricle bases. Rotation of the auricle is not sufficient in this case.
We remove a narrow spindle-shaped strip of cartilage via the approach described above. If possible, an additional incision on the front should be avoided. However, we detach the skin from the cartilage, as this allows the skin to shrink and prevents the formation of skin folds by our plastic surgeons in Düsseldorf.
Ears for extreme concha measurements
Here we perform the ear removal in Düsseldorf as explained in the previous section. However, the piece of cartilage to be removed is significantly larger. Therefore, we cannot do without an additional scar on the front at the base of the pinna. The skin would otherwise leave clearly visible wrinkles. We therefore also have to remove skin.
In the case of large ears (medical macrotia), the above-mentioned procedures can be extended to include ear reduction if desired. Our plastic surgeons make the incision at the edge of the ear (inner edge of the helix) and remove the crescent-shaped excess. The scar is usually barely visible.
