Ear reduction (or auricle reduction, ear reduction) can be used to correct ears that are too large. The medical term for large ears is macrotia. The transitions are fluid, the relative size depends on body size and head size. The approximate normal value of the axial length is between 58 and 62 mm for women and between 62 and 66 mm for men. The indication for ear reduction is usually based on the psychological suffering of those affected, which is often exacerbated by jokes and teasing.

What is the procedure for ear reduction?

Ear reduction can be performed on an outpatient basis under local anesthesia, possibly with additional twilight sleep. General anesthesia is recommended for anxious adults or children. Before the procedure, the hair and ears must be washed thoroughly to reduce the germ rate preoperatively. After ear reduction, a larger cotton wrap bandage is only applied for 2-3 days. After that, a smaller bandage with a loose-fitting headband is usually sufficient.

How is ear reduction performed for macrotia?

Di Martino first described a method of ear reduction back in 1856. It was a wedge cut technique, similar to the one shown in the 1st and 2nd picture above. Many other descriptions followed. Today, various techniques are used that are accompanied by well-hidden, very inconspicuous scars (the scarring can be seen in the pictures above).

The planning of the operation first involves a careful examination of the ears. The choice of surgical technique depends on the areas that require correction. It may also make sense to combine ear reduction with ear pinning and/or earlobe correction. Such combination operations cost slightly more than the individual procedures.

The principle of the operation is to remove the excess skin and cartilage in order to create a natural-looking ear. An accurate suturing technique is crucial for a result with inconspicuous scars and a pinna without steps. However, adequate patient cooperation should not be underestimated in order to avoid tensile forces on the fine sutures after ear reduction.
